History (HX): Left hemiparesis for 2 days . male78y Findings:Abnormal signal in right MCA territory high signal on DWI and low on ADC map Diagnose (DX):Acute right middle cerebral artery infarct involving the precentral gyrus Discussion: DWI is very sensitive and specific technique for cerebral ischemia in the first 3 hours after the onset of ischemia. The duration of restricted diffusion (low ADC, bright on DWI) is about 10 to 14 days. The ADC then normalizes (pseudonormalization) following that it becomes elevated (encephalomalacia) |
